MediRoutes deserves credit for usage-aligned pricing and for publishing it: $150 a month plus 50 cents a trip on Tier 1 with one broker integration, or $250 a month with unlimited integrations [2]. Now read its own billing rules: a roundtrip counts as two managed trips, and a no-show or a cancel at the door, where your driver already showed up, bills as a performed trip [3]. On the payer mixes where no-shows cluster, that's a meaningful surcharge on your worst days. Add the base fee that starts before your first trip, the $100 fee for a second broker integration on Tier 1, per-use notification charges, a 3.5% card surcharge, and a $35 paper-check fee [2].

Duet's meter has one moving part: the loaded time of the trips you run. No base fee, free under 100 trips a month, empty return miles and deadhead never billed, the AI agent and rider texting included, billing monthly in arrears, and most providers under $50 per active vehicle per month [1]. Eighteen years in market is a genuine asset, and operators we talk to often inherited MediRoutes with the business and kept it running for years. But age shows. A customer CEO wrote in a February 2025 public review that the platform "seems to be lagging in the industry," and recent reviews report rate increases arriving without a meeting and IVR calls billed on top of the subscription [7]. MediRoutes shipped rider notifications and a facility portal earlier than most of the field, and both are real. But its "Where's My Ride" links and reminder notifications bill per use [3][4], and as of August 2026 there's no AI phone-answering product in its published materials [4]. Duet includes the whole demand-side stack at no extra cost: a 24/7 AI phone agent that answers, books, and reminds, tracking links that drove about 15% more repeat bookings and 45% fewer status calls for providers who send them (Duet customer data, 2026), and a facility portal [1]. And beyond the software, the growth team: facility outreach calling, Google Ads, and websites. MediRoutes sells software; Duet's job is your trip count.

When MediRoutes is the better pick

If your team has years of fluency in it and no acute pain, switching costs are real and staying can be rational; we'll say that to your face on a demo. Its MAS API also matters if your book runs through New York Medicaid, where Duet doesn't have a native integration today. And operations built deep into its billing workflows, like the automated Modivcare Ride Receipt billing its own partner page describes, have a workflow reason to stay. If none of those describe you, and especially if the tool "came with the business," you're not choosing MediRoutes; you're just not choosing yet.
